PROVISION OF PWD IDENTIFICATION CARD
Republic Act 9442 “Magna Carta for PWD” and its implementing rules and regulations state the issuance of the identification card to persons with disabilities by the local government units. The said Identification Card will be beneficial to the sector of person with disabilities in obtaining the benefits and privileges entitled them under R.A 9442 or the Magna Carta for Person with Disabilities.
CLIENT:
Person with Disabilities
REQUIREMENTS:
Barangay Clearance
Certificate from the doctor or attending a physician indicating the nature of client’s disability.
1 x 1 id picture
Process of Availing the Service
Accomplish the application form. Ensure the correctness of the data entered.
Provide the newly issued Person with Disability Card and prepare the purchase booklet to be utilized in the purchase of medicine. Ensure the signature of the PWD in the logbook.
PROVISION OF ASSISTIVE DEVICES
One of the sectors that the MSWDO are mandated to provide with basic services are the person with disability (PWD) such as that of the visually impaired, hearing impaired, and the physically handicapped. Provision of assistive devices will enable them to perform tasks and responsibilities similar to that of an able individual.
CLIENT:
Person with disabilities ages 59 years old and below regardless of disabilities. He/She must bea resident of San Juan, Batangas.
REQUIREMENTS:
Barangay Clearance
Certification from the doctor or attending physician indicating the need for such assistive devices.
Process of Availing the Service
Assist the client in signing the client’s logbook.
Inquire on the purpose of the client’s visit on the agency.
Conducts an interview using the application form for person with disabilities and the intake sheet.
Prepares needed documents necessary on availing the service.
Prepares the social case study report and other relevant documents (such as picture, medical report, and recommendation for assistive devices) to be submitted on the agency concerned.
