ASSISTANCE TO INDIVIDUAL IN CRISIS SITUATION


The Assistance to Individuals in Crisis Situation (AICS) is part of the MSWD’s protective services for the poor, marginalized and vulnerable/disadvantaged individuals. Through the MSWD, it provides direct financial assistance which is defined as limited assistance, either in case or guarantee letters (to service providers) to individuals and families whose normal functioning has been hampered due to difficult situations brought about by dysfunctional situations that may be caused by poor health conditions, natural and man-made calamities, and other crisis situations.

Who can avail the assistance?

The beneficiaries shall be the walk-in and referred clients. Priority shall be given to:


1st - Indigent families

2nd - Families belonging to the informal sector

3rd - Other poor, marginalized, and vulnerable/disadvantaged families


AVAILABLE FORMS OF ASSISTANCE

Medical Assistance

Assistance to help shoulder hospitalization expenses, purchase of medicines, and other medical treatment (i.e. laboratory procedures including but not limited to CT scan, ECG, 2D Echo, assistive medical devices, etc.) and other medical expenses.


REQUIREMENTS:

  • Cinical Abstract / Medical Certificate with signature and license number of the attending physician (issued within 6 months)

  • Hospital Bill (for payment of hospital bill), or Prescription (for medicines) or Laboratory requests (for procedures)

  • Barangay Clearance/Certificate of Indigency

  • Community Tax Certificate

  • Letter request


Burial Assistance

Assistance to shoulder funeral costs and related expenses, including, but not limited to, cases of bringing home the bodies of their deceased loved ones.


REQUIREMENTS:

  • Registered Death Certificate

  • Barangay Clearance

  • Community Tax Certificate

  • Letter request


Educational Assistance


Assistance given to eligible beneficiaries to help defray the other cost of sending students/children to school such as school tuition fees, school supplies, transportation allowance and other school related expenses. The type, kind, and amount of financial assistance to be provided to beneficiaries shall be based on the assessment and recommendation of the MSWD social workers and/or other authorized personnel, dully approved by the authorized official/s of the MSWD.


REQUIREMENTS:

  • Enrolment Assessment Form or Certificate of Enrolment or Registration

  • School ID of the student/beneficiary

  • Barangay Clearance or Certificate of Indigency

  • Community Tax Certificate

  • Letter request

A client can avail of the assistance only once within three (3) months. Availment of assistance beyond this limit must be supported by a justification based on the special need of the beneficiary per assessment of the social worker.

Process of Availing the Program

  • Instruct the client to fill in the logbook as proof of availing the services

  • Accomplish the AICS forms to enable the clients to avail the necessary assistance as identified and requested.

  • Attached all the required documents

  • Prepare disbursement vouchers and refer the clients to the Office of Municipal Accountant and Treasurer’s Office for the release of financial assistance.

REFERRALS FOR EXPANDED AICS OF DSWD REGIONAL OFFICE

It provides ranges of service such as: Immediate rescue, provision of direct financial assistance, psychosocial support, material assistance including medical, transportation, financial, burial and other services.


CLIENTS:

  • Walk in clients

  • Referred clients of the CIU’s in DSWD CO, FOs, and/or satellite offices in the provinces.

  • Priority shall be given to the following:

  1. Beneficiaries belonging to indigent families under National Household System of Poverty Reduction (NHTS-PR) ;

  2. Families belonging to the informal sector;

  3. Other poor, marginalized and vulnerable/disadvantage families.

Medical Assistance

  1. Clinical Abstract/Medical Certificate with signature and license number of the attending physician procedures (issued within 3 months).

  2. Hospital bill (for payment of hospital bill) or prescription (for medicines) or laboratory request for procedures).

  3. Barangay Certificate or any valid ID of the client.

  4. Referral Letter ( if applicable).

  5. Of 10,000.00 and above a case study report is needed.


Burial Assistance

  1. Funeral Contract;

  2. Registered Death Certificate;

  3. Barangay Certificate or any valid ID of the client;

  4. Permit to transfer /Health permit (for transfer of cadaver);

  5. Referral letter if (applicable).

Transportation Assistance


  1. Brgy. Certificate or any valid ID of the client;

  2. Police Blotter ( for victims of pick pockets, Illegal Recruitment, etc.);

  3. Referral Letter (if applicable).

Educational Assistance

  1. Barangay Certificate or any valid ID of the client.

  2. Enrollment Assessment form or Certificate of Enrollment or Registration;

  3. School ID of the student/beneficiary;

  4. Referral Letter (if applicable)

Process of Availing the Service

  • Accomplish the GIS or PSP form.

  • Secures other signatures required.

  • For referrals, prepares the necessary papers and forward to the DSWD Region IV – A for funding and releases of assistance.

  • Let the client signed the client’s logbook as proof of availing the service.

EMERGENCY RELIEF ASSISTANCE

PROVISION OF DISASTER RELIEF ASSISTANCE

The Municipal Social Welfare and Development Office is at the forefront of relief assistance during natural or manmade calamities such as typhoon, fires, and earthquake. Among others, it distributes relief goods and provides housing materials to families whose houses have been destroyed by calamities.

MSWDO also provides financial assistance and referrals to other agencies providing emergency/relief assistance.

CLIENTS:

  • Victims of natural or man-made calamities.

REQUIREMENTS:

  • Barangay Certification testifying that he/she is a victim of calamity.

  • BFP Certification testifying he/she is a victim of fire.

  • Picture of damaged house

Process of Availing the Service

  • Sign the client’s log book as proof availing the service.

  • Brief client on the service and its requirements.

  • Conducts an interview and accomplishes the AICS or ESA form.

  • Prepares a disbursement voucher and refers client to the Municipal Accounting and Treasurers Office for the release of the financial assistance.

CERTIFICATE OF INDIGENCY

PROVISION OF CERTIFICATE OF INDIGENCY

A Certificate of Indigency is required to avail of the services of charitable institution, government offices and non-government organizations and institutions.

CLIENT:

  • Indigent individuals/families

REQUIREMENTS:

  • Barangay Certification of Residency

  • Certificate of Indigency from the barangay where the client is residing.

  • Certification from Municipal Assessor’s Office that the party concerned does not own real property.

Process of Availing the Service

  • Briefs client on the service and its requirements.

  • Conduct interview.

  • Prepares Certificate of Indigency.

  • Issue Certificate of Indigency.

SOCIAL CASE STUDY REPORT

PROVISION OF SOCIAL CASE STUDY REPORT

Social Case Study Reports are required by charitable institutions, government hospitals and non-government organization that provide social services to indigent client and patients.

REQUIREMENTS:

For referral to PCSO and other funding agencies

  • Medical Certificate or Medical Abstract

  • Hospital Bill

Process of Availing the Service

  • Instruct client to fill in the log book.

  • Conduct interview and determines the case if the case requires a home visit for further validation.

  • If home visit is not necessary, prepares the Case Study Report. Provides the client a copy.

  • Schedule the home visit with a client.

  • Obtain the signature of the MSWDO. Release Case Study.
